EHCP Hub

All students who attend Foxwood Academy will have an Educational Health Care Plan (EHCP). EHCPs identify educational, health and social needs and set out the additional support to meet those needs. 

The plan is reviewed annually within Foxwood Academy by healthcare professionals, teachers, the young person and their family/care team to consider what outcomes they would like to achieve and identify what is needed to achieve those outcomes.

From January 2022, Nottinghamshire County Council have introduced the EHC Hub which is a new digital approach to creating and maintain EHCPs.  The hub provides a secure platform for families, young people and practitioners to make their contribution to the development of EHC Plans.

Where is my EHCP? - We've noticed that it can be difficult to access your EHCP on certain types of mobile device. If you ever need a paper copy of your EHCP please contact us at school and we can send you a paper copy of the document. 

I can’t make the meeting? – We try to make sure that your class teacher/tutor is at the review meeting. We feel that these are the best people to help review targets and set targets for the coming year.  Please try to give us plenty of notice if you need to postpone your meeting so that we can re-arrange our cover in school.

If you can’t make the meeting in person we can complete the session virtually (over Zoom or Teams) or over the telephone. Please contact us at school to discuss the options.
